1)	Since we will be using the weight of your body AS A PRIMARY FITNESS TOOL, I need to know if you weigh:

2)	How does it feel when you walk a block?
	I feel just fine. I could speed walk for half-hour at a time with no problem. I feel just fine. I could walk (average speed) for a half-hourat a time with no problem. I feel fine, but I'm glad I don't have to walk much further. I have aches and pains when I walk, but I can do it. I am quite out of breath when I walk a block, but I can do it. I cannot walk a block, but I can walk shorter distances. I cannot walk from my house to the street, forget about walking a block. I walk around my house leaning on things and balancing myself as I go. I cannot walk because of my large body size. I cannot walk because of a disability and I use a wheelchair.

3)	Do you cope with any of these medical conditions? (Check all that apply.)

4)	Can you get up and down off of the floor?
	Sure, no problem. I can, but it hurts my knees. No way! Even if I got down, I could never get back up! This question does not apply to me because I use a wheel chair.

5)	Does it bother your neck when you do sit-up type exercises (or do you simply dislike doing sit-ups)?
	Yes No

6)	Do you prefer to do your aerobic work sitting or standing?
	Sitting Standing
